Titanium All-on-x Surgery Techniques, Placement and Options

Understanding that titanium implants, which represent 99.9% of the full arch dental cases, have been used by dentists for full mouth reconstruction and implant-supported prosthetics for decades, have certain advantages: They are manufactured by over 20 implant manufacturers and come in all shapes, sizes, and connections, and have what is known as “custom abutments,” which allow the titanium implants to be far more forgiving in the hands of a dentist. 

Titanium dental implants are almost exclusively placed as 3-piece implants – which means a small and tolerable mistake in surgical placement (both location and angle) of titanium implant can easily be fixed with a custom titanium abutment, which will even out the missed angle or the slightly skewed placement. 

Surgeons and implant dentists are ONLY trained to place titanium implants and so it is the most even those willing to try zirconia dental implants don’t know how to work with the single one piece design or parallel placement, making it an unrealistic undertaking for a conventional dentist. 

Now that we’ve covered titanium implant advantages being 

1) they are mass-produced by over 20 manufacturers and have unlimited variations to their technical lineups and product selection 

2) they have the ability to have custom abutments made to make the case easier to restore or to fix any less than perfect implant placement, offering a forgiving solution to conventional surgeons and implant dentists 

3) which allows them to be used as angled implants (angled means not parallel in the mouth) since the custom abutments can be angled in a reverse direction to achieve the appropriate angulation with which to attach a prosthetic.

Ceramic Implant All-on-X Surgery Techniques, Placement and Restorative Options

Zirconia implants, which are placed by ONLY the highest skilled biological surgical dentists like Dr. May, are more expensive, costing north of $800 per zirconia implant; difficult and less forgiving to place and restore, requiring precise surgical technique that allows for zero margin of error; and requiring more bone and grafting. While zirconia implants have been approved in the USA by the FDA since 2007 and have over 20 years of research, there are still limited manufacturers compared to metal implants. Almost all of Dr. May’s full arch zirconia implant cases use SDS Zirconia Implants, with a mix of one piece and two piece implant types. 100% of his placement is parallel, unlike the angled nuances of titanium implants, making zirconia implant placement and restorations much like natural teeth (which are parallel in the jaw).

It’s important to understand that while zirconia dental implants are VERY strong, they have less flexural give than a metal alloy implant. This limits the ceramic implants to being manufactured with very little angulation, and almost all zirconia dental implants are made completely straight. 

Zirconia implants also have very specific restorative limitations that again, are mastered by Dr. May, but many conventional dentists would have a difficult time restoring a full arch of zirconia dental implants without the flexibility of titanium abutments with angulation and customization options. What this means is zirconia implants in a full arch must be placed perfectly “parallel”; otherwise, the restorative aspects will be extremely difficult and increase the likelihood of failure and zirconia bridge breakage (in addition to implant failure long term).

23. Governing Law; Venue (CT-centric; harmonized with arbitration)

These Terms are governed by Connecticut law, without regard to conflict-of-laws rules. For any court action permitted under Section 23 (e.g., small claims or injunctive relief), exclusive venue is state or federal courts located in Connecticut, and you consent to jurisdiction there.

24. Limitation of Liability (Max lawful; Site-only; explicit clinical carve-out)

To the fullest extent permitted by law, the Practice and related parties will not be liable for indirect, incidental, consequential, special, exemplary, or punitive damages, or loss of profits/data/goodwill arising from Site use.

Clinical care carve-out: These limitations apply to Site Disputes and Site use, not to professional liability arising from in-office clinical care.

25) Binding Arbitration; Class Action Waiver (Site/Media Disputes Only)

PLEASE READ — AFFECTS YOUR RIGHTS. Except where prohibited by law, any dispute, claim, or controversy arising out of or relating to the Site or these Terms (“Site Dispute”) will be resolved by binding individual arbitration and not in court.
